Position Title:    Clinician, Community Support Services Team

Responsible To:      Outreach Clinical Supervisor

Qualifications:    LCSW with supervisors credentials required and a Master’s degree from an accredited university.

Responsibilities:

  1. Demonstrate a holistic understanding of adults with mental illness and regard them as persons with dignity and competence, engaging them as full partners in service planning, delivery and evaluation.
  2. Establish positive, caring and trusting relationships with consumers.
  3. Foster consumers’ independent thinking; use a strengths based wellness and recovery model.
  4. Participate in a multi-disciplinary team for the purpose of evaluating consumers referred for placement, providing support to consumers residing in the Community Support Services and Apartment Programs, assessing client strengths/needs and helping consumers formulate an individualized service plan.
  5. Complete the Rehabilitative Needs Assessment and Individualized Rehabilitation Plans for program participants
  6. Develop an awareness of case management needs for consumers in order to support case management and counseling staff with their responsibilities in service delivery (case management, advocacy, illness management and recovery, daily living skills enhancement, etc.)
  7. Provide specialized services in accordance with individual disciplines and as permitted by licensure and as indicated in Individualized Rehabilitation Plans (e.g. medical device education, psychotherapy, etc.)
  8. Effectively access and employ community resources:  identify, develop and maintain good relationship and linkage with a wide range of community resources.
  9. Participate as a partner in the CSS Leader Team including operating as an advisor for the Team Leader
  10. Ensure all consumers receive regular outreach services according to their needs and service plans.
  11. Help consumers to access community resources, facilitate and support natural support networks.
  12. Support consumer’s efforts to manage symptoms, using the Illness Management and Recovery Model.
  13. Provide transportation as needed to access and maintain linkage to community services (e.g., medical, psychiatric, social services, vocational).
  14. Provide out of office individual supportive counseling, crisis intervention, case management and advocacy services to assigned case load and as support and back-up to case management staff.
  15. Participate in the Housing Services Clinical Team by attending scheduled meetings, trainings, and working collaboratively to support the needs of the department.
  16. Liaison with other service providers for the purpose of promoting continuity of care.
  17. Provide Crisis Intervention Services
  18. Maintain documentation as related to direct service.
  19. Document progress of consumers in progress notes and logs according to requirements of regulatory agencies
  20. Conduct activities in a culturally competent manner, ensures clients rights and responsibilities, adhere to confidentiality rules.
  21. Participate in all appropriate staff, supervision, committee and training meetings
  22. Assume other duties as assigned by the Outreach Clinical Supervisor/Chief Operating Officer
