SAIL Case Manager (Pequannock, NJ)

Position Title:    Outreach Case Manager – NewBridge SAIL (Senior Assistance for Independent Living) 

Responsible To:  SAIL Team Leader

Qualifications: Bachelors Degree in Social Work, Psychology.  Experience in programs for seniors or community outreach/case management activities preferred.  Valid NJ driver’s license.  Ability to manage time independently.  Good communication and organizational skills

Responsibilities:

  • Provide phone consultation to community callers regarding resources available to area seniors and family members.
  • Uses phone assessment skills to determine need for further services.
  • Provides outreach services through home visits, contact with significant others and other relevant persons to meet the client’s needs.
  • Assesses client needs and abilities and provides supportive counseling, advocacy, and follow-up to ensure successful linkage with other services/agencies.
  • Coordinates with other service providers for the purpose of promoting continuity of care within the least restrictive environment.
  • Provides transportation of clients (as needed) to various medical and social service appointments within the community as needed, using only NewBridge Services vehicles as authorized by supervisor.
  • Collects and electronically enters data and maintains documentation per regulatory authorities.
  • Maintains a current file on community resources for client services.
  • Conducts quality assurance data collection using client satisfaction survey.
  • Collaborates with supervisor to ensure accuracy of service data.
  • Attends monthly staff and supervision meetings, and collaborates with interdisciplinary team.
  • Other duties as assigned.
